Team Member Health Insurance Benefits

Moonlight Pizza & Brewpub is proud to offer two distinct health insurance options for eligible team members. Employees may choose one option during the enrollment period based on their needs.

General Health Insurance

This option provides traditional health insurance coverage for employees and eligible dependents.

Eligibility

  • Employees working an average of 32 hours per week are eligible.

  • Hours taken off must be made up to maintain this average.

Enrollment Period

  • Eligible employees may enroll in one of the following ways:

    • During open enrollment (Nov 15 to Dec 15 for the following year)

    • Within 60 days of being hired

    • After a qualifying life event (e.g., marriage, birth, loss of other coverage)
      Enrollment is only available during these times.

  3. Receive plan details and pricing.

  4. Select a plan and complete the enrollment process.

Employer Contribution

  • Moonlight contributes $200 per month toward the employee’s chosen plan.

  • For dependents, Moonlight contributes $100 per month per dependent (up to 2 dependents).

Direct Primary Care (DPC)

This option provides affordable, direct access to a primary care physician for employees.

Eligibility

  • Available to any employee after 60 days of employment, working a minimum of one day per week.

Enrollment Period

  • Eligible employees may enroll at any time after completing 60 days of employment.

    • No open enrollment window—available year-round

    • May also enroll after a qualifying life event

Employer Contribution

  • Moonlight covers 50% of the $90/month fee for eligible employees.

  • Managers and Shift Leaders receive 100% employer coverage for their monthly fee.

Frequently Asked Questions

  • This only applies to General Health Insurance. Eligibility is based on an average of 32 hours per week. If you take time off, you’ll need to make up the hours in other weeks to maintain eligibility. DPC does not have a weekly hour requirement, only that you work at least one shift per week.

  • No, employees must choose only one option during the enrollment period.

    • General Health Insurance: Available after 2 months of employment (or during open enrollment).

    • Direct Primary Care (DPC): Available after 60 days of employment, even if you only work one day per week.

  • You can only switch plans during the annual open enrollment period (Nov 15 – Dec 15) or after a qualifying life event (marriage, birth, loss of other coverage).

  • Only if you are enrolling in General Health Insurance. Our broker (Tasha Briggs and her assistant Kat) will help you understand your plan options. Speaking with them does not obligate you to sign up.

    If you are choosing Direct Primary Care (DPC), you only need to notify Tori, and she will guide you through the enrollment process.
