Moonlight Pizza & Brewpub is proud to offer health insurance coverage to eligible team members. Enrollment is available during the designated enrollment period, with benefits designed to support your health and well-being throughout the year.

General Health Insurance

This option provides traditional health insurance coverage for employees and eligible dependents.

Eligibility

Employees working an average of 32 hours per week are eligible.
Hours taken off must be made up to maintain this average.

Enrollment Period

Eligible employees may enroll in one of the following ways:
- During open enrollment (Nov 15 to Dec 15 for the following year)
- Within 60 days of being hired
- After a qualifying life event (e.g., marriage, birth, loss of other coverage)
  Enrollment is only available during these times.

Receive plan details and pricing.
Select a plan and complete the enrollment process.

Employer Contribution

Moonlight contributes $200 per month toward the employee’s chosen plan.
For dependents, Moonlight contributes $100 per month per dependent (up to 2 dependents).

Frequently Asked Questions

This only applies to General Health Insurance. Eligibility is based on an average of 32 hours per week. If you take time off, you’ll need to make up the hours in other weeks to maintain eligibility.
General Health Insurance: Available after 2 months of employment (or during open enrollment).
You can only make changes during the annual open enrollment period (Nov 15–Dec 15) or after a qualifying life event, such as marriage, birth, or loss of other coverage.
